RN CASE MANAGER
A Registered Nurse who is responsible for planning, coordinating, providing and documenting care for a geographically assigned caseload of patients from day of admission through day of discharge. Utilizing physicians’ orders and professional skills, the Case Manager RN develops and implements a home health plan that meets each patient’s specific needs and is in compliance with TJC, federal and state regulations, reimbursement guidelines as well as Agency policies and procedures.
LICENSED PRACTICAL NURSE
Provides skilled nursing care to patients in their home under the supervision of a Registered Nurse. Provides service in accordance with the Agency philosophy, standards and policies. Provides skilled nursing in the home as ordered on the plan of treatment and nursing care plan.
HOME HEALTH AIDE
Performs duties essentially of personal care which might be given to convalescent, chronically ill or disabled persons in their homes. Attempts in every possible way, in working with individuals and families, to assist them to assume responsibilities toward self-care. Promotes patients to engage in positive, healthy activities whenever possible, and educates loved ones in proper assistance techniques.
MEDICAL SOCIAL WORKER
The role of the Medical Social Worker provides psychosocial assessments, diagnosis and treatment to patients in their place of residence as prescribed by a physician, to assist the patient/family member(s)/caregiver(s) develop effective coping skills in response to the patient’s diagnosis, treatment and/or life situation.
The role of the Office Specialist is to promptly, efficiently and accurately execute all aspects of the referral process and on-line authorization of routine services, contacting specialty care providers, monitoring patient eligibility and performing on-line data entry of routine authorizations/denials.
This position requires attention-to-detail and the ability to handle multiple administrative responsibilities and is an extension of our clinical team, which assists our nurses with the administrative functions of obtaining authorizations to support patient goals.
